SYMPTOM(s):
SYSTEM ERROR: bkfradd: block is already on chain <number>. (419)
FACT(s) (Environment):
HP-UX 11 32-bit
Progress 8.3B
CAUSE:
User connected to the database issued a ctrl-c within the session and was disconnected.
When the user attempted to reconnect, the user was notified a that the transaction was already active and was disconnected.
The user had the system admin perform a kill -9 against the PID of the user's earlier session.
The database then abnormally shutdown because that session was holding buffer locks.
When the database was restarted, the (419) error occurred.
FIX:
An index build may be attempted but may not successfully remove the block from the chain.
A more definite fix would be to dump and load.
