FORM HEADER - adds an extra line before form print and after
**** DO NOT MAIL TO CUSTOMER ********************
When printing FORM HEADERS -
PROGRESS automatically puts 1 extra line in before the top header
prints and one extra line after the bottom header prints.
This is internal to progress.
You cannot get rid of the extra line.
THIS IS AN FYI
If you send the form to a file, vi that file and do a set nu,
you will see the line numbers and notice form starts on line 2 instead
of line 1.
