SYMPTOM(s):
AppServer processes hang on shutdown.
AppServer processes do not appear to terminate as expected on shutdown.
AppServer Server processes cannot be terminated or killed without rebooting the server machine.
AppServer Server processes become a child of init (parent PID of 1)
FACT(s) (Environment):
IBM AIX 6.1
OpenEdge 10.1C
CHANGE:
Migrated from 9.1D09 to 10.1C04 and issue is seen immediately after migration.
CAUSE:
This is a IBM AIX Bug IZ94396: THERE IS A TIMING ISSUE BETWEEN THE SYNC DAEMON AND A MAPPED FILE.
Applies to AIX 6.1 TL-5 or above.

After rebooting the server (after upgrading the SP), see if the level is correct and AIX APAR IZ94396 if is installed:
# oslevel -s
# instfix -ik IZ94396
