Blank XML namespace attributes in XML documents
Using XML DOM parser (X-DOCUMENT and X-NODEREF objects)
XML nodes created without namespace reference
xlmns attributes not explicitly added to nodes
Nodes that have other attributes will include a blank xmlns atribute when the XML document is SAVE()'d

The exact cause is unknown at the time of this writing
FIX:
Upgrade to OpenEdge 10.2B01 or later
The issue is not seen in later releases
