Database is started by the AdminServer
Running dbtool against the database
Closing command prompt window/session while dbtool is still running/connected

CAUSE:
Bug# OE00192591
FIX:
Upgrade to 10.2B01.
A workaround is to avoid closing the command window for a running/connected dbtool session.
