CAUSE:
This problem occurred when users were members of active directory operating units. This is, at its root, a security issue but was directly caused by something in the setup of the Operating Unit.
FIX:
Since the exact parameters of the cause are unknown there is no way to provide a definitive fix, however by switching the users to an alternate (Clean) Operating Unit the problem went away.
