SYMPTOM(s):
When loading a WSDL document which contains a 'form' attribute within an 'xsd:element' tag, the following exception is thrown:
Error loading WSDL document http://webserver/WSCustomize/wrapped.asmx?WSDL : http://webserver/WSCustomize/wrapped.asmx?WSDL:92: error: 'form' attribute can only be used if 'type' or 'ref' is present on 's:element' declaration (11748)
CAUSE:
Bug# OE00107909
FIX:
Upgrade to OpenEdge 10.1B or later.

As a workaround remove the 'form' attributes from the WSDL document.
