CAUSE:
This is might be related to an oracle bug according to an Oracle Faq found on the Internet.
FIX:
Make sure Oracle version is at least 9.2.0.8 (Server Patch Set) ,10.1.0.5 (Server Patch Set) ,10.2.0.2 (Server Patch Set)
