CAUSE:
The cause is unknown at the time of this writing
FIX:
none at this time. However, there are workarounds:
1. If the problem is occuring on Windows2000, upgrade to Service Pack 4
2. If the problem is occuring on Windows XP, upgrade to Service Pack 2
