A dbrpr scan may or may not show errors.
Copying the database files using operating system tools sometimes fails.
Backing up the database sometimes fails.

CAUSE:
Hardware failure causing corruption.
Occasionally the database files may be accessed without receiving errors, this is because some hardware faults are intermittent. Often, harddisk faults are like this and the harddisk can float in and out of proper working order before total failure.
The most likely sources of this kind of corruption is a faulty harddisk, faulty harddisk controller, or faulty memory.
FIX:
Contact your hardware vendors and have them check the hardware for faults.
Note that checking hardware logs is NOT enough as they often come out clean when in fact there is an underlying fault, you must have the hardware vendor perform low level testing of the components.
