AppBuilder Palette -> "SDO" button -> "New" button.

FIX:
Option #1
Upgrade to OpenEdge 10.1C or later
Option #2
Create the SmartDataObject using AppBuilder -> press "New" button and select "Static SmartDataObject (SDO)".
Option #3
Create the SmartDataObject using AppBuilder menu -> File -> New -> and select "Static SmartDataObject (SDO)".

Go to AppBuilder -> Tools -> Administration -> File -> Preferences
Make sure the "Custom templates" and "Custom palettes" Fill-in's are set to '*'
