FACT(s) (Environment):

SYMPTOM(s):
can't create database with DataDictionary
Cannot find or open file <dbname.db>, errno = 2. (43)
trying to create database with prodb causes error.
Error creating _dbutil process 193
prostrct create does not cause any errors
Environment Variables are correct, there is enough disk space, permissions are correct.
CHANGE:
Customer installed Service Pack 08 for Progress 9.1D
CAUSE:
Windows Error 193 is "not a valid Windows application". The size and "last modified date" for _dbutil.exe in the DLC/bin directory are not consistent with what it should be for _dbutil.exe from 9.1D08 This executable is corrupted, perhaps due to a media problem, or it was changed on disk.
FIX:
Reinstall the correct and clean _dbutil.exe executable.
