The table has a unique index on 3 component fields, two _obj fields and the third a character field. But the 132 error is suggesting that only two components (the _obj fields) on the unique index are being checked without considering the 3rd character component of the index.
CAUSE:
The error is due to the fact that the entity_key_field in the entity_mnemonic table only had the first 2 of the 3 fields of the unique index.
FIX:
Use the Entity Control tool to modify the entity key field for the table entity mnemonic, adding the missing key component.
