SYSTEM ERROR: I/O error 6 in readfrmb, ret -1, file 6 (lbi) (srt), addr 81905664.
Examining the OS log file /var/adm/messages shows disk access errors
CAUSE:
Disk failure.
FIX:
Replace the failing disk and restore the database from the last valid backup.
