NT Performance Monitor prevents proserve startup 8.1A (and higher)

If the NT Performance Monitor is being run on a database started with
Procontrol and then the database is stopped, subsequent attempts to
start a server for the database will fail.
Version 8.1A:

The failure message depends on the method used to start the server and
the version of Windows NT being used:

ProControl on NT 3.51
"Can't Find Lock (.lk) file"
The Application Event log will contain the message:
"Unable to create shared memory...."

ProControl on NT 4.0
STOP dialog box with the message:
"The instruction at "0x00473cb0" referenced memory at "0x0000003e".
The memory could not be read".

proserve from the DOS command line
"Unable to create shared memory....(1719)"

Workaround is to shut down the Performance Monitor before attempting
to re-start the server.
