Read-only files on NT, errno 5

DRAFT COPY - Currently under review and edit.

On Windows NT with Win32 processes, if you try to access a read-only
file on read-write mode, you will get errno 5, and not errno as it
happens with 16-bit processes.

This situation was seen with a customer that had his database files
set read-only and tried to startup a NT DB server. He would get
PROGRESS error 43 with errno 5 instead of 13.
