Adding a record within a table (with CREATE statement)
Table created from within Roundtable (by _update.w program)
CAUSE:
Starbase has a reproducible case but they have not found yet why this problem happens on a specific table
FIX:
Run "proutil <db> -C idxbuild" for the index _file-num of the table _file.
