CAUSE:
Provision Plus comes with WebSpeed Workshop, which has a 2 agent license limit. The default setting for maxSrvrInstance and initialSrvrInstance in in ubroker.properties is 5.
FIX:
In ubroker.properties file or through Progress explorer, set maxSrvrInstance and initialSrvrInstance equal to the licensed agent count, 2.
