Field Name fill-in, Apply button and access radio set at the bottom right hand corner are hidden in:
Administration
Security Control (treeview)
+Security Allocation (text node)
+Fields (node)
Switching from another node in the treeview and coming back to field node
CAUSE:
Known issue, development aware. This particular folder window makes this problem when the session caches dynamic frames for treeviews.
FIX:
Choose the Filter tab and choose Detail tab again
or
Choose the Refresh button
or
Resize slightly the treeview container Window.
