SYMPTOM(s):
Error running Prostrct add
SYSTEM ERROR: bkiotbl index <number> is invalid. (2414)
Placing new extents in a different directory
Directory located on a newly created file system
CAUSE:
There is a problem with the newly created file system. Possible configuration issue.
FIX:
1) Unmount file system.
2) Run fcheck to scan and fix problem.
3) Remount file system.
4) Add additional extents.
