Format of extent <extent> failed at byte <byte>. (6751)
Error occurs during random extents upon subsequent retries.
CAUSE:
Filesystem and/or hardware corruption has occurred.
It's possible that operating system error logs show no errors.
FIX:
Inform your hardware vendor to perform both a hardware check and a filesystem check.
