The LD_LIBRARY_PATH has not been set correctly
FIX:
Set the LD_LIBRARY_PATH as per Fathom HA Clusters release notes - change LD_LIBRARY_PATH to include $PSC_CLUSTER_PATH/lib and $PSC_CLUSTER_PATH/bin.
