Database comes down with no errors
No errors in Event Viewer Logs
No errors in Database log file
User activity shown in log file stops abruptly with no indication of an abnormal shutdown event.
.lk file remains behind

CAUSE:
This behavior may occur if the database was started by one user and someone logs that user off the system using Start -> Shut Down -> Log Off User Name or CTRL+ALT+DEL -> Log Off.
FIX:
Do not log the user who started the database off while the database is running.
