I/O error <n> in <program>, ret <n>, file <n>, addr <n>. (1676)
I/O error 5 in fputcxdll, ret -1, file 28, addr 0. (1676)
fputcxdll indicates character put into a buffered stream.

Power outage that required the running FSCK to bring the server back up.
CAUSE:
Bad sectors on the hard drive.
FIX:
Replace the hard drive.
