CHANGE:
Upgraded to version 9.1D
CAUSE:
This is a known issue - the bulkload session in v9 is not started with "-i" option, despite the fact that it's being stated in the dbname.lg file
FIX:
Start version 9 bulkoad with option "-i".
$ proutil dbname -C bulkload dbname.fd -i
