Removed shared memory with segment_id: 139

Did NOT truncate the BI (proutil <db name> -C truncate bi) with the old version before upgrading
CHANGE:
Upgraded to 9.1D
CAUSE:
One cause could be that the BI was not truncated.
FIX:
Restore from backup.
